ANGEL’S CARE HOME HEALTH is a Chicagoland, Medicare certified, home health care agency.  We are always looking to add professionals of multiple disciplines (RN, PT, HHA, and OT) to our growing team.  We are patient focused and committed to delivering quality and personalized home care. 

 

Founded and operated by two lifelong nurses, our team has been at work since 2012, delivering skilled home care to home-bound, Medicare covered patients across the city.  Our office is conveniently located near the Kennedy Expressway in Chicago’s Irving Park neighborhood and the door is always open (well, at least from 9am-5pm).

These positions are part-time with the potential for full-time opportunities. 

We are interested in bringing qualified professionals of several disciplines to our team:

 

• Registered Nurse

• Home Health Aide

• Physical Therapist

• Occupational Therapist

 

Ideal candidates must display a strong sense of professionalism, integrity, and enthusiasm.  Communication is at the forefront of our work environment and candidates must be a dedicated team player and exhibit zero lapses in communication.  Candidates must also demonstrate strong writing skills.  As a member of the staff you will make visits on your own time on a weekly basis to patients while providing visit notes and communicating with our administration, physician, and patients.  Experience in Home Health Care is a huge plus!  Diversity is highly embraced and candidates of all backgrounds are highly encouraged to apply as we broaden our patient scope. 

 

 

You will be responsible for evaluating your patient and administering the plan of care and follow-up visits, documented properly, on a weekly basis using Axxess software.

 

We offer competitive pay-per-visit rates, which will be covered should the application process continue.

 

Requirements

• Ability and willingness to travel

• 2 years of experience in the nursing/therapeutic field (any more is a plus)

• Home Health experience a huge plus

• Valid certifications and licenses from the State of Illinois

• References (at least 1)

• Health Examination (Physical, TB, Vaccinations)

• CPR license

• Ability to work in a team environment
